Request for a copy of a Birth Certificate

The cost for a certified copy of a birth certificate in Princeton is 25.00 each.

All requests MUST include a copy of a current picture identification of the applicant AND a copy of the address page of a utility bill, car registration card or car insurance card showing the applicants current address.

If your last name has changed from childhood (or if the applicant is the mother and your name has changed since the child’s birth) due to marriage or legal action, please submit a copy of the appropriate documentary proof, such as marriage certificate or court documents.

You will be prompted to attach these documents at the end of the application process.

If the person you are requesting a birth certificate for was born at the Princeton Hospital after May 2012, they may not have been born in Princeton, NJ. If they were born at the University Medical Center of Princeton at Plainsboro-DO NOT SUBMIT A REQUEST. You must call Plainsboro at 609-799-0909, Township Clerk's office, to request a birth certificate.

Your request will be processed within 7-10 business days. If you need the birth certificate before that time please call the Health Department 609-497-7608 to make those arrangements do not submit an online request.

Please upload the following to be included with your application a current copy of a government-issued photo ID of the applicant.  If your Drivers License does not include a photo and you do not have a passport, include with the copy of the license a copy of 2 of your most recent utility bills (no older than 60 days) which show the name and address listed on the license. ​Also include a copy of the address page from a current utility bill, car registration, or car insurance card.
